141: Assessment of the Newborn

This series provides a comprehensive introduction to care of the term newborn.  It includes five individual programs, each approximately 20 minutes in length.  Viewing this series will provide individuals with an understanding of the issues related to the special needs of the newborn and impart knowledge of a wide variety of congenital issues and anomalies.  The viewer will be presented with both accurate clinical information and current regulatory guidelines. 

These programs provide a virtual experience of the actual assessment and care through the use of footage taken in an actual birthing room and newborn nursery.  Anatomical information on internal organs is supported with clear graphic depictions.  Hospitals will find the series invaluable for the orientation of new staff to the nursery and obstetrical department, while nursing programs will have the opportunity to provide students with a virtual experience in this demanding field of nursing.

141.1 The First Ten Minutes (16 min.)

1. List the three primary roles of the placenta and umbilical cord during pregnancy.
2. Describe the role of surfactant in the lungs.
3. Discuss current understanding of the factors which foster the newborns first breath.
4. Delineate steps to be taken if there is meconium in the amniotic fluid.
5. Discuss the four routes of heat loss and the role of brown fat.
6. List seven factors which increase a newborn’s susceptibility to cold stress.
7. Explain the parameters of the APGAR score.

141.2 Head to Toe Assessment (15 min.)

1. State the two appropriate methods for taking the emperature of a neonate.
2. State the normal ranges of neonatal vital signs.
3. Give the reason for eye prophylaxis.
4. Discuss routine cord care
5. List the normal ranges for newborn weight, length, head and chest circumference.
6. Describe the proper way to measure the newborn.
7. List the steps in the head to toe external exam.
8. Discuss the process of newborn screening.

141.3 Cardiopulmonary Assessment and Cardiac Anomalies (15 min.)

1. Describe the ways in which fetal circulation varies from that of the adult.
2. List the three types of respiratory retractions.
3. Differentiate between transient tachypnea of the newborn and respiratory distress syndrome.
4. List and describe the four categories of cardiac defects.
5. Name the eight most common cardiac congenital anomalies.

141.4 Physical Anomalies and Neurologic Issues (15 min.)

1. Discuss the differences between major and minor anomalies and the significance of two or more minor anomalies.
2. Name the factors related to the increase in brain size during the first year of life.
3. Compare and contrast the types of neural tube defects.
4. Describe the immediate intervention for an infant with gastroschisis.
5. List the four known risk factors for neonatal hypoglycemia.
6. List five maternal factors that can predispose the newborn to hypoglycemia.

141.5 Gestational Age and Newborn Reflexes (15 min.)

1. Give the ages at which the New Ballard Gestational Age assessment is performed.
2. List the steps in the Neuromuscular Maturity portion of the Ballard exam.
3. Describe the seven parameters observed in the Physical 5. Maturity portion of the Ballard exam.
5. Name and describe 13 newborn reflexes.
